Introduction to Care Planning and Health Assessments

This session will provide practice nurses new to care planning with an overview of criteria, guidelines and eligibility for GPMPs, health assessments and using templates in general practice.

This introductory education session is provided through the Practice Nurse Support Program (PNSP) supporting nurses new to general practice and is a recurring session.
